We are excited to provide our patients easy-to-use online digital forms to save time and the environment!

New patients, we are excited to have you as part of our dental family. Our front desk team will contact you via your preferred contact method (email or text) with directions and any necessary forms that need to be completed before your first appointment.

Below are a few examples of information that we will need:

• Personal information; name, address, phone number, email address, etc
• Dental insurance information
• Medical and dental history

For our current patients, when needed our office will periodically contact you via your preferred contact method (email or text) with directions and the necessary forms that need to be update.

Below are a few examples of why your forms may need updated:

• Experiencing a change in your overall health since your last visit
• Taking new prescriptions or over the counter supplements
• Have had any recent surgeries, especially pertaining to the heart or joints
